Forms and Waivers
It is important to complete the form accurately:
If your child has special needs such as allergies, phobias, or medical alerts, including asthma, diabetes, or epilepsy. Include this information with home and work phone numbers on the form provided.
Forms must be complete before the start of class. Your child will not be able to participate without these forms.
