By completing this form, you are providing information regarding alleged misconduct by an employee. An investigation will be initiated based on the information provided below. Please be advised that false reporting may result in disciplinary action for the reporter. This is a confidential matter; please protect the integrity of this investigation by not discussing it with anyone outside of the Human Resources Department.

Once the form is received, the issue will be investigated, and appropriate action is taken, which may or may not include contacting the employee, you, or any witnesses you have identified. Our goal is to help employees proactively address individual challenges that may hinder their success at Elgin Community College, and if applicable, connect them with the appropriate resources.
